You do not need to be an established Lassen Medical Group patient.
To become a Lassen Medical Group patient, phone 530-527-0414, Ext 168, and leave a message. In a short period of time, you will be contacted.
To speed the process, you may print the New Patient Request Form, fill it out and return it to Lassen Medical Group. It can be faxed, mailed, or you may drop it off with any of our receptionists.
